The COVID-19 vaccine is now available for all children Age 12+. The U.S. Food and Drug Administration (FDA) and the Centers for Disease Control and Prevention (CDC) recently approved the use of the Pfizer COVID-19 vaccine for all children age 12 and older.
Jewel-Osco will host a vaccine clinic for anyone age 12 or older at Central Middle School located at 18146 Oak Park Avenue, Tinley Park on Saturday, May 22. Interested persons age 12 or older can sign up now.
Osco Pharmacy will provide the Pfizer vaccine which requires 2 doses. The first dose will be administered on Saturday, May 22, and the second dose will be on Saturday, June 12 at Central Middle School. Osco Pharmacy requires that you commit to receiving both doses on these dates in order to participate in this vaccine clinic.
All populations in Illinois age 12 or older can receive the vaccine regardless of insurance coverage or immigration status, including individuals who are undocumented. No fee will be charged to the patient for this vaccine. Jewel-Osco vaccination clinic is entirely voluntary, the vaccine is not a requirement for students to attend Kirby School District 140, the District does not have any responsibility for the Clinic.
Minors age 12-15 must be accompanied by a parent or legal guardian to receive a vaccine. The parent /guardian is required to sign the informed consent form as well as legibly print name and relationship to the child. Use the Jewel-Osco vaccination registration portal below to register for your appointment. Appointments are available beginning at 9 a.m. on Saturday, May 22. Second dose appointments are guaranteed on Saturday, June 12 at the same location and the same time of your first dose.
